Student Application
The application for the Career Development & Mentorship Program includes information about both the student and a parent or guardian. The program is free for accepted students.
Required information
- Student’s full name, date of birth, current grade, school name, and city
- Student’s email and phone number
- Parent or guardian’s name, relationship to the student, phone, and email
- Preferred contact method (phone, email, or text)
- Career pathway selection
- Short answers: why you want to join, the career you’re most interested in, your goals after high school, and a challenge you have overcome
- Eligibility confirmation: satisfactory grades, willingness to attend monthly mentorship sessions, and understanding that attendance and respectful behavior are required
- Today’s date
Optional
- Most recent report card (PDF or photo)
- Up to two recommendation letters (a letter, email, or other message from a reference)
- Parent/guardian signature on the participation permission statement
